Frequently Asked Questions

Do I have to identify myself when submitting a report?

While we encourage you to provide contact information so we can follow up with you, we understand concerns about retaliation. We protect reporter identity to the extent permitted by law.

How long does an investigation take?

Investigation timelines vary based on complexity and the type of concern. Emergency matters are prioritized for rapid response, while standard matters may take several weeks to months. You will be updated on your report status.

What happens after I submit my report?

Your report is logged, assigned a reference number, triaged for urgency, and assigned to an investigator. You will be contacted for any follow-up information needed. A findings report and any corrective actions will be documented.

Can I be retaliated against for submitting a report?

No. Retaliation against anyone for submitting an oversight report is illegal and itself subject to investigation. Report any retaliation attempts directly to us.

What if I don't have evidence to support my concern?

You should provide your best account of what happened. While supporting documentation is helpful, we investigate based on what you report. We may discover additional evidence during our investigation.
